Intake & Assessment Program
The Intake Worker provides one-on-one client support and creates action plans based on survivor’s unique situations. The Intake Worker then provides referrals to the Disability Assistance, Rehabilitation & Care Management and Group Support Programs, where appropriate.
Process for Intake
v Gather initial information from the person with an ABI and /or
their family.
v Identify the general needs of the person with an ABI and /or their family.
v Provide information about NBIS services.
v Determine whether or not the NBIS services are a suitable match for the person with an ABI and/or their family.
v Ensure that if NBIS services are not a suitable match, the person with ABI will be bridged to the most appropriate services.

The Outreach Rehabilitation Worker creates plans to reflect the changing levels and amounts of support required, by focusing on promoting independence. They address daily living activities, which include household, financial and time management; personal and family support; communication skills; social and leisure activities; cognitive deficits; physical needs; vocational and emotional needs; and community access and integration. Together, with the client, the Outreach Rehabilitation Worker focuses on enhancing personal strengths and respecting the clients’ individuality.

v Learn and develop new strategies to adjust to changes in their thinking capabilities.

v Ensuring consistent communication between all team members, the survivor, caregivers and the funding agency.
v Developing partnerships within the community in order to bridge some of the existing gaps.

Disability Assistance Program (DAP)
The Disability Assistance Worker provides assistance in advocacy, assessing resources, debt management and in applying for provincial and federal financial benefits.

Education Prevention and Awareness Program
v A significant number of Acquired Brain Injuries are preventable.
v The most cost effective approach dealing with ABI is to invest in its prevention.
v NBIS distributes materials on education, prevention and awareness to various locations in the community including medical clinics, schools, drug and alcohol services and mental health agencies.
v We are also available to speak with groups about the issues and barriers of ABI and its preventions.
v Information tables with posters and brochures are set up at various events through out the year.
